Hospitals in charge have to connect to the drug controller directly with the following details.
Checklist to procure Amphotericin B in Karnataka
The Prescription should be made on the letterhead of the hospital.
The following particulars should be mentioned:
- The Doctor’s name with KMC Registration, date, signature & seal. No proxies should be used.
- All patient particulars including name, age, gender, etc.
- A clear diagnosis mentioning
a) The type of mucormycosis
b) CT finding and or Biopsy result - Procedure or Surgery, if any, done on the patient.
- Clearly mention:
a) Whether Liposomal/Emulsion/Lyophilised Amphotericin B is required
b) Dosage for the patient including the number of vials needed per day
c) Duration of the treatment - Aadhar card of the patient to be enclosed.
